Setiap pekerjaan tidak pernah terlepas dari potensi bahaya dan risiko keselamatan dan kesehatan kerja. Preliminary Hazard Analysis (PHA) merupakan upaya pencegahan dan pengendalian risiko terhadap kecelakaan kerja. Fishbone diagram merupakan metode yang digunakan untuk menghubungkan faktor-faktor yang berpengaruh dalam suatu proses dengan berfokus pada akar penyebab masalah. Tujuan penelitian untuk mengidentifikasi risiko keselamatan dan kesehatan kerja pada pengoperasian incinerator di RSUD Prof. Dr. H. Aloei Saboe Kota Gorontalo.
Desain penelitian ini merupakan desain deskriptif dengan pendekatan mixed method. Informan penelitian terdiri dari 1 informan kunci, 1 informan utama dan 1 informan pendukung. Pengumpulan data melalui observasi, wawancara terstruktur dan dokumentasi.
Hasil penelitian bahwa pada setiap pekerjaan di area pengoperasian incinerator berpotensi bahaya mekanis, listrik, biologi, kimia, dan ergonomi. Hasil penilaian risiko didapatkan 1 risiko tertinggi sehingga memerlukan tindakan lanjutan dan 60 risiko terendah sehingga risiko dapat diterima. Selanjutnya dilakukan identifikasi faktor penyebab risiko tertinggi didapatkan 31 akar penyebab permasalahan. Sehingga dilakukan rekomendasi pengendalian terhadap faktor penyebab risiko tertinggi dan didapatkan 10 rekomendasi pengendalian administratif.
Simpulan bahwa terdapat 1 risiko tertinggi yaitu cedera ringan/berat akibat tidak menggunakan APD lengkap. Saran kepada RSUD Prof. Dr. H. Aloei Saboe Kota Gorontalo untuk dapat melakukan pengendalian risiko dengan memperhatikan aspek pada setiap bagian pekerjaan sehingga pengendalian yang dilakukan tepat sasaran dan kepada pekerja untuk dapat mematuhi peraturan pengendalian risiko yang diberikan rumah sakit.

Every job must be having potential hazards and risks to occupational health and safety. Carrying out Preliminary Hazard Analysis (PHA) is one effort to prevent and control the risk of work accidents. A Fishbone Diagram is a method used to connect the influencing factors in a process by focusing on the cause of the problem. This study aims to identify the occupational health and safety risks in the operation of incinerators at RSUD Prof. Dr. H. Aloei Saboe, Gorontalo City.
This is a descriptive study using a mixed method approach. The informants consist of 1 key informan, 1 main informant, and 1 supporting informant. The data collection technique uses observation, structured interview, and documentation.
The finding shows that every job in the incinerator operating area has the potential, for mechanical, electrical, biological, chemical, and ergonomic hazards. The result of study on risks obtains 1 highest risk so that it requires further action and 60 lowest risk that is still acceptable. Furthermore, identification of the factors causing the highest risk obtains 31 root causes of the problem. Therefore, a recommendation is made to control the factors causing the highest risk and results in 10 administrative control recommendations.
The conclusion is that there is 1 highest risk, which is minor/severe injury due to not using complete Personal Protective Equipment. Suggestion to RSUD Prof. Dr. H. Aloei Saboe, Gorontalo City is to be able to carry out risk control by paying attention to aspects of each part of the work so that the control carried out is right on target and for workers to be able to comply with the risk control regulations given by the hospital.
