Nursing documentation is the recording and reporting of evidence belonging to the nurse in performing nursing posts in the interest of clients, caregivers and health team in providing health services to caregivers responsibility.
The purpose of this study was to analyze the factors relating to the implementation of the nursing care process documentation in Space RSUD. Surgeon Prof Dr. Saboe Aloei Gorontalo City.
Cross-sectional study design to implementing nurse population is directly involved in the application process and documentation of nursing care nursing care process. total extraction of samples Sampling on 32 respondents. and techniques for data collection using questionnaires. processing the data using SPSS Version 16 computer presented in table form and narrative.
Analysis results show that the implementation of the nursing care process documentation in RSUD Assoc. Dr.. Saboe Aloei Gorontalo city is highly associated with self-knowledge and the availability of nurse guide / documentation format. even reward factors, psychology and social work environment and availability of the proportion of time was also related to the implementation of a test documentation but no statistically significant association.
Organized advice to colleagues seprofesi that in carrying out even found the factors that inhibit implements process documentation, as professional nurses let these factors do not hinder us to keep doing nursing care process documentation and

Dokumentasi keperawatan merupakan bukti pencatatan dan pelaporan yang dimiliki perawat dalam melakukan catatan perawatan yang berguna untuk kepentingan klien, perawat dan tim kesehatan dalam memberikan pelayanan kesehatan dengan tanggung jawab perawat. Tujuan penelitian ini adalah untuk menganalisis faktor-faktor yang berhubungan dengan pelaksanaan pendokumentasian proses asuhan keperawatan di Ruang Bedah RSUD Prof. Dr. Aloei Saboe Kota Gorontalo.
Desain penelitian cross sectional dengan populasi adalah perawat pelaksana yang terlibat langsung dalam penerapan proses asuhan keperawatan dan pendokumentasian proses asuhan keperawatan. penarikan sampel total sampling pada 32 respoden. dan teknik pengumpulan data dengan menggunakan kuesioner. pengolahan data menggunakan kumputer program spss versi 16 disajikan dalam bentuk tabel dan narasi.
Dari hasil analisis data menunjukan bahwa pelaksanaan pendokumentasian proses asuhan keperawatan di RSUD Prof. Dr. Aloei Saboe Kota Gorontalo sangat berhubungan dengan pengetahuan perawat itu sendiri dan tersedianya panduan/format pendokumentasian. walaupun faktor reward, psikologi dan sosial lingkungan kerja dan tersedianya waktu secara proporsi juga berhubungan dengan pelaksanaan pendokumentasian namun secara uji statistik tidak ada hubungan yang bermakna.
Saran yang dianjurkan kepada rekan-rekan seprofesi agar dalam melaksanakan tugas walaupun ditemukan faktor-faktor yang menghambat dilaksanakannya proses pendokumentasian, sebagai perawat profesional hendaknya faktor-faktor tersebut tidak menghalangi kita untuk tetap melakukan pendokumentasian proses asuhan keperawatan mengingat pentingnya dan bermanfaatnya proses pendokumentasian tersebut.
