Febriyanti A. Ishak, 811418087. 2022. Analisis Tinjauan Proses Pengelolaan Berkas Rekam Medis Rawat Jalan Di Puskesmas Kota Selatan. Skripsi, Pembimbing I Dr. Sylva Flora Tarigan, M.Kes dan Pembimbing II Ramly Abudi, S. Psi, M.Kes. Jurusan Kesehatan Masyarakat, Fakultas Olahraga Dan Kesehatan, Universitas Negeri Gorontalo.
Misfile (Filling) dokumen rekam medis menjadi hal perlu diperhatikan karena jika dokumen rekam medis hilang atau rusak akan menyebabkan kelambatan dalam proses penginputan data pasien untuk menyiapkan dokumen rekam medisnya. Hasil Penelitian Assembling didapatkan untuk formulirnya sudah sesuai dengan SOP, Coding di dapatkan untuk klasifikasi penyakit tenaga rekam medis tidak ada yang tahu yang berwenang hanyalah dokter. Indexing / tabulasi hanya menggunakan 1 lembar kertas , Filling Terjadi Misfile karena factor man, money, method dan material. Sistem Penjajaran dokumen menggunakan straight numenical filling Contohnya seperti 01,02,03,04,05. penyusutan (retensi) dan pemusnahan berkas rekam medis untuk memilah dokumen yang aktif dan tidak aktif itu ada dan untuk pemusnahan berkas rekam medis itu menggunakan mesin pencacah atau penghancur kertas dokumen rekam medis. diperoleh kesimpulan Assembling untuk sudah sesuai SOP, Coding klasifikasi penyakit yang tahu hanya dokter, Indexing di muat dalam 1 lembar, Filling terjadinya misfile disebabkan oleh faktor man, faktor money, faktor method dan faktor material. Sistem Penjajaran menggunakan sistem penjajaran langsung atau straight numenical filling, Retensi Dan Pemusnahan Rekam Medis Untuk Retensi memilah dokumen aktif dan in aktif itu ada.

The act of Misfile (filing) of medical records needs more attention. It is because, during the process of inputting the data, it is very crucial to prepare it with no delays. Therefore, the medical record cannot be lost or damaged. In regards to that, the results of the Assembling Research found that the form was in accordance with the SOP, and Coding was obtained for the classification of diseases. No one knew about it except the only authorized doctors. Moreover, the Indexing/tabulation only used 1 sheet of paper, and Misfile occurred due to man, money, method, and material factors. The document alignment system used straight numerical filing, for example, 01, 02, 03, 04, 05. On the other hand, depreciation (retention) and destruction of medical records in sorting out the active and inactive documents as well as destroying them, was done using a paper shredder. Therefore, it can be concluded the assembling was in accordance with the SOP, coding for the disease classification was known only by the doctors, indexing was loaded in 1 sheet, and the misfile in the filing process was caused by financial, money, method, and material factors. In addition, the alignment system used a direct alignment or straight numerical filing system, whereas the retention and destruction of medical records in sorting out the active and inactive documents existed.
